Driver Vehicle Inspection Report (DVIR)
The record a driver makes of a vehicle condition check, typically at the start or end of a shift. Where defects are supposed to enter the maintenance system.
A driver inspection report records what the operator found: lights, brakes, tires, fluids, damage. Where a defect is noted, there is an expectation that it is repaired or assessed before the vehicle goes back out.
The failure mode is not the inspection, it is the handoff. Reports that go into a folder rather than into the maintenance system produce a fleet where drivers report faults, nothing visibly happens, and reporting quietly stops. Fleets that get value from DVIRs are the ones where a defect becomes a work order automatically and the driver can see that it did.
It is also the cheapest early warning a fleet has. The operator sees the vehicle every day and the shop sees it a few times a year.
